Marc Mayer, D.O.

Dr. Mayer has been with Avenel Iselin Medical Group for over 30 years practicing family medicine. In addition, he is President of the medical group.
Dr. Marc, as he is referred to at the office, graduated from Fairleigh Dickinson University with a B.S. in Biology/Chemistry and then went on to Midwestern University – Chicago College of Osteopathic Medicine, where he received his D.O. degree. He then served his residency in family medicine at the University of Medicine and Dentistry NJ (UMDNJ).
Dr. Marc explains, “Our office, and especially the Department of Family Medicine, is committed to the Patient-Centered Medical Home (PCMH) model of health care. Our mantra is to provide accessible, comprehensive, and coordinated healthcare to our patients.”
Dr. Marc has been a member of the Adult Value-Based Program’s Advisory Board for Horizon Blue Cross/Blue Shield, and is a delegate to the New Jersey Association of Osteopathic Physicians & Surgeons, on the staff of Hackensack Meridian JFK Medical Center, and Robert Wood Johnson Medical Center/Rahway.
Recently, the Mayer Family welcomed their first grandchild Charlotte Joelle.
Mitchell Mayer, D.O.

Dr. Mayer is a board-certified physician who has been with Avenel Iselin Medical Group for 30 years. He serves as Vice-President of the group, focusing on family medicine.
In 1981, Dr. Mitch completed his undergraduate studies at Fairleigh Dickinson University, followed by completing his medical school degree at The Chicago College of Osteopathic Medicine, where he earned a Doctorate of Osteopathy degree in 1985. His residency program was completed at UMDNJ, Kennedy Memorial Hospital, in 1988.
Dr. Mitch’s philosophy of practice is encompassed by the PCMH model adopted by the medical group which focuses on improved quality of care by coordinating care and empowering patients with the knowledge to improve their overall health.
Neil Perilstein, M.D.

Dr. Perilstein joined Avenel-Iselin Medical Group in 1995, where he now serves as Corporate Secretary of the Avenel Iselin Medical Group, with a focus on family medicine.
Dr. Perilstein attended Georgetown University, where he graduated from in 1988, followed by Georgetown University School of Medicine, where he graduated from in 1992 with a Medical Doctorate degree. He is a family practice physician who joined AIMG in 1995, directly out of his residency program at Overlook Family Practice, a Columbia University affiliate.
Dr. Perilstein enjoys a variety of activities in his free time, such as playing the guitar, surfing, cycling, and photography.
Alicia Lubin-Baskin, D.O.

Dr. Lubin-Baskin has been with Avenel-Iselin Medical Group since 2009, with a focus on family medicine. She graduated from the University of Osteopathic Medicine and Health Sciences in Des Moines, IA in 1990. Prior to joining AIMG, Dr. Lubin-Baskin practiced on Staten Island for 13 years.
Jo Ann Colbert, PA-C
Physician Assistant
Family Medicine
Jo Ann Colbert graduated from Colorado State University with a Bachelor of Science as a Physician Assistant. She also has extensive experience as a hospitalist and surgical Physician Assistant.
In 1999, Ms. Colbert joined Avenel Iselin Medical Group as part of the Family Medicine group.
When she is not caring for patients at AIMG, she enjoys spending time with her four children as well as hiking and vacationing in the mountains.
Marina Grundman, PA-C
Physician Assistant

Marina Grundman is a nationally certified Physician Assistant (PA) with vast experience in family practice, internal medicine, and as a Hospitalist PA. She earned her Bachelor of Science Physician Assistant degree from D’Youville College in Buffalo, NY, and completed her PA training at Buffalo General Hospital as well as Erie County Medical Center.
Before joining AIMG’s family practice group in 2005, Ms. Grundman worked as a Hospitalist PA at Rochester General Hospital in Rochester, NY. Ms. Grundman is trilingual, speaking English, Russian, and Latvian.
Prior to immigrating to the United States from Latvia in 1991, Ms. Grundman was a classical musician. She holds a Master of Arts in musicology from the Latvian State Academy of Music, which she earned in 1985.
Outside of work, Ms. Grundman enjoys spending time with her family, traveling, classical music, opera, theater, art, and gardening. She also loves animals–namely her cats Ema and Matilda, and her golden retriever grand-dog, Waffles.
Kristina Mancini, PA-C

Kristina Mancini graduated with a Bachelor’s Degree in Biology from Seton Hall University in 2009, followed by a Master’s Degree from Seton Hall University’s PA Program in 2011. She joined AIMG shortly after graduating in 2011 and is a member of the Academy of Physician Assistants. She enjoys the close relationships she has built with her patients and their families.
Anyone who knows her knows that pink is her color of choice! In her free time, she enjoys spending time with her family and friends.

Swati Gupta, M.D.

Dr. Gupta received her undergraduate degree in 2005 from Johns Hopkins University with a major in Cellular & Molecular Biology. In 2012, she graduated from Rutgers University – RWJMS with a degree in internal medicine. In 2015, she completed her residency at Robert Wood Johnson Medical School and is now board certified by the American Board of Internal Medicine.
When Dr. Gupta is not treating patients at AIMG, some of her hobbies include catching a good read, cooking up a great meal, and traveling.

Anjum Tanwir, M.D.
Cardiology
Dr. Tanwir grew up in Brooklyn, New York, and attended the Brooklyn College CUNY University of New York. He completed his medical studies in internal medicine and general cardiology at Newark Beth Israel Medical Center. He then finished his medical training at the University of Massachusetts in interventional cardiac and vascular training in 2010.
Dr. Tanwir is board-certified in internal medicine, cardiovascular disease, nuclear medicine, cardiology, and interventional cardiology. He practices all aspects of cardiology and vascular medicine with a passion that is unconditional.
This father of two has beautiful girls who keep him on his toes all the time! His hobbies include extreme sports such as skateboarding, skiing, rock climbing, and snowboarding. Other interests are the beach, traveling, and summer in general.
Mayank Patel, M.D.
Gastroenterology
Dr. Patel has been with AIMG for over ten years, serving in the Gastroenterology department.
Dr. Patel is a Board-Certified Internal Medicine physician who specializes in the field of Gastroenterology. He graduated from Mahatma Gandhi Memorial Medical College in Jamshedpur, India in 1978 earning an MBBS degree. In 1981, he became a Medical Doctor with permanent certification in the United States. He completed his residency program at Jersey City Medical Center, an affiliate of UMDNJ in 1983 where he was the Chief Resident
Robert Czyzewski, M.D.
Nephrology/Hospitalist
Dr. Czyzewski leads the Nephrology department at AIMG. He has over 30 years of experience in the medical field and is notable across the region as an authority in treating glomerulonephritis & nephropathy.
Dr. Czyzewski also serves as a Hospitalist for patients who must be admitted to a hospital.
Sang Song, D.O.
Physiatrist
Dr. Song has more than 20 years of experience in osteopathic medicine and is board certified in physical medicine and rehabilitation and certified by the National Board of Osteopathic Medical Examiners.
He graduated from Johns Hopkins University in 1995 with a Bachelor of Arts in Psychology and then went on to the UMDNJ School of Osteopathic Medicine from which he graduated as a Doctor of Osteopathic Medicine in 2000. Dr. Song completed his residency in physical medicine and rehabilitation at JFK Johnson Rehabilitation Institute in 2004.
Aqsa Siddiqui, D.P.M.
Podiatry Services
Dr. Siddiqui received her undergraduate degree in 2007 from Rutgers, Newark with a double major in biology and psychology. In 2012, she graduated from the New York College of Podiatric Medicine specializing in foot and ankle surgery. In 2015, she completed her residency at Trinitas Hospital in Elizabeth, NJ. Graduating in 2016, her fellowship took place at St. John’s Hospital in New York with a dual specialty in advanced wound care and tissue healing.
Dr. Siddiqui is board certified by the American Board of Podiatric Medicine and does minimally invasive surgery of the foot and ankle. In 2022, Dr. Siddiqui received a Top Doctor award in Podiatric Medicine.
When she is not caring for patients, she enjoys spending time with her husband and doing arts & crafts with her 2-year-old daughter and 5-year-old son.
Adam Kopelan, M.D., FACS
General Surgery
Dr. Kopelan is the Chairman for the Dept. of Surgery and Director of Surgical Services at Newark Beth Israel Medical Center.
Dr. Kopelan earned his medical degree from New York University School of Medicine, New York, NY. He received his postgraduate training in General Surgery at the University of Chicago Hospitals where he also was a Research Fellow in Immunology. His undergraduate degree is from Gettysburg College, Gettysburg, PA.
Board certified in General Surgery, Dr. Kopelan has published articles and presented on topics including Robotics and Minimally Invasive Surgery, Transplantation and Vascular Surgery. His practice interests are in Advanced Minimally Invasive, Bariatric, and Endocrine Surgery utilizing laparoscopic, robotic and endoluminal techniques. Dr. Kopelan has a particular focus in resident and medical student education. In 2003 he was the recipient of the Robert Baker Golden Apple Teaching Award at the University of Chicago, and in 2005 he received the Golden Apple Teaching Award at Monmouth Medical Center. He is currently a member of Phi Beta Kappa, the Society of Alimentary and Gastrointestinal Surgery, and the Society of Bariatric and Metabolic Surgeons.

David Pertsemlidis, M.D., FACS
General Surgery
Dr. Pertsemlidis was born and raised in Manhattan, he attended the Dalton School and Princeton University, graduating magna cum laude with a degree in American history. After attending the New York University School of Medicine, he completed his residency in general surgery at the New York University Medical Center. He has published several publications on laparoscopic, pancreatic, and adrenal surgery, most recently in a comprehensive endocrine surgery textbook. His practice includes laparoscopic and general surgery with a focus on the endocrine system, the gallbladder and bile ducts, gastric and colon cancer, the gastrointestinal organs, and abdominal wall hernias. He is currently the Director for the Department of General Surgery at the Newark Beth Israel Medical Center. Previously, he was an Assistant Clinical Professor of Surgery at the Mount Sinai School of Medicine and an Attending Surgeon at The Mount Sinai Hospital and Morristown Medical Center. Dr. Pertsemlidis is certified by the American Board of Surgery and is a Fellow of the American College of Surgeons.

Esther Pollack, LCSW
Behavioral Health
Esther Pollack has been with AIMG since 2017. She received a Bachelor of Arts with a major in psychology and a minor in sociology from Clark University in 1990. She graduated from New York University in 1992 with a Master of Social Work, MSW.
Her specialties are working with children, adolescents, and adults experiencing anxiety and depression. In 1998, she received a special certification in the New Hope Guild postgraduate Child and Adolescent Therapy Program.
In her free time, she enjoys spending time with family and friends, reading, and doing yoga. She also enjoys going to Broadway Musicals.
